​​​​​​​​​​Thank you for your interest in scheduling an appointment with the Orthopedic Medicine and Mobility​ ​(OMM) service. We suggest you bookmark this web page, as it has the information you’ll need to get scheduled and items you may want to reference later.

Is Your Pet a New OMM Patient?

Your pet is considered a new patient with our service if any of the following are true:

  • Your pet has never been seen by the Orthopedic Medicine and Mobility (OMM) service.
  • It has been at least 12 months since your pet's last visit with the OMM service.
  • The OMM service has not seen your pet for the current orthopedic problem he/she is experiencing.
If your pet is considered a new patient based on the criteria above, please complete our pre-appointment survey to schedule an appointment. The website resources provided in the survey may be useful to you and your pet at a later time, so the same resources are also listed below for future reference.

The pre-appointment survey describes the types of appointments you can choose from (an in-hospital exam or telemedicine consult) and gathers client information and important information about your pet’s problems. It also provides a link through which you can upload videos of your pet’s gait (and provides instructions on how to do so) and then directs you to our online scheduling system so you can set up an appointment.

If your pet is a new patient, then completing our pre-appointment survey is all you need to schedule an appointment.

Recheck Appointments

If your pet does NOT meet the criteria above to be considered a new patient and you are trying to schedule a recheck appointment, please email us at Please know that we are currently experiencing a very high volume of phone calls and emails. It may take seven (7) business days before we can return your message. We apologize for this delay. 

​If your pet is experiencing an emergency (such a​s a fractured limb), please call the hospital at (970) 297-5000 and ask for our Urgent Care service.

While completing the survey, you will be required to sign the two consent forms below if you choose to schedule a telemedicine appointment. For in-hospital appointments, you will be provided with these forms at the time of your visit. They are provided here simply for your information. Please note that some items on these forms may relate more to in-hospital visits, but at this time, we are using the same forms for both telemedicine and in-hospital appointments.
