Program Eligibility & Enrollment
Our program is designed to help low-income elderly or disabled pet owners living in Larimer County keep their pets at home for as long as possible.

Eligibility Criteria

  1. Financial Eligibility: Provide Medicare, Medicaid, W2 or other evidence that pet owner is low-income. (Eligible clients are those with monthly gross income not greater than $1,765.00; annual income no greater than $21,180 for a one person household).
  3. The pet owner must be disabled OR aged 60 or older.
  5. The pet owner must live in Larimer County.

Enrollment Process

​To enroll or inquire about the program, contact Dr. Lori Kogan at or (970) 491-7984.
An initial phone screening with the pet owner or caretaker is conducted to determine eligibility and whether Pets Forever can meet the requests of the pet owner.
After a phone screening, an extended intake at the pet owner's home is scheduled. A Pets Forever representative visits the pet owner's home, meets both the owner and the pet(s), and gathers additional information and paperwork.
After this step is completed, the owner is enrolled as a Pets Forever client and is then paired up with one or more volunteers for requested services.

If you find a stray animal, please call the
Larimer Humane Society, 3501 E​. 74th St., Loveland, (Map​​), (970) 226-3647.

Please DO NOT bring stray animals to the hospital during these hours:
Monday-Friday 8 a.m.-6 p.m.
Saturday-Sunday 8 a.m.-4 p.m.

The hospital will accept stray animals when the Humane Society is closed:
Monday-Friday 6 p.m.-8 a.m.
Saturday-Sunday 4 p.m.-8 a.m.​​​
