Graduate School Forms at-a-glance: Steps to Graduation

GS6

Event: First year of study concluded or at discretion of advisor

Done by/Signed by: Student and their advisor/Associate Department Head

Submit to: Heidi Runge

Notes: Serves as an academic plan for the student - also allows the Graduate School to know who is on a student's committee. Must be submitted before registration for 4th semester, failure to submit will result in a registration hold for the student.

GS9A

Event: Committee member changes prior to Preliminary Exam, only required if committee members change.

Done by/Signed by: Student and their advisor/Associate Department Head

Submit to: Heidi Runge

Notes: This form MUST be done prior to taking prelims if there have been any changes in the student's committee - failure to do this could cause students to have to take prelims all over again! This does not apply to the temporary substitution policy for Preliminary Exams.

GS16

Event: Preliminary exam for PhD students

Done by/Signed by: Student's committee generates/signed by all committee members/Association Department Head

Submit to: Heidi Runge

Notes: Provides the Graduate Dchool with the results of a student's preliminary examination. MUST be submitted at least two semesters prior to final defense AND the results submitted within two days after the results are known. The completed, signed GS16 form MUST be submitted at the graduate school within 2 days of exam.

Student must notify the Department (Heidi Runge​) at least 3 weeks in advance of Preliminary Exam, notification should include name of substitute committee member for the exam.

GS24

Event: Student has defended

Done by/Signed by: Student's Committee

Submit to: Heidi Runge

Notes: Notifies the Graduate School that a student has defended.  Department must announce student's defense at least two weeks prior to the event. The completed, signed GS24 form MUST be submitted at the graduate school within 2 days of the defense.

GS25

Event: Student is in his/her last semester at CSU

Done by/Signed by: Student generates/advisor and Associate Department Head

Submit to: Heidi Runge

Notes: Notifies the Graduate School that a student plans to graduate. Strictly-enforced deadlines apply and are published in advance by the Graduate School.

Re-apply for Graduation

Event: Student didn't graduate by intended semester but has already submitted a GS25 form previously

Done by/Signed by: Student generates and submits electronically

Submit to: Graduate School

Notes: Notifies the Graduate School that a student has previously submitted an application for graduation form (GS25) but is planning to graduate in a later semester. Strictly-enforced deadlines apply and are published in advance by the Graduate School.

GS25B

Event: Department knows that all requirements for graduation have been met.

Notes: Not needed for MIP students.


GS52

Event: Student has been notified of discrepancies in her/his application for graduation.

Notes: Rectifies any discrepancies between the GS6 (Course of Study) and GS25 (application for graduation) forms.​​​​
